Kwan-Yin Healing Arts Center made the difficult decision to cease operations on June 30th, 2026. We are grateful to all the patients who walked through our doors and the providers who offered their services for the last twenty-five years.

We are pleased that Nourishing Life Collective has opened in our former Northeast Sandy location and has become your medical records custodian. You will find some familiar faces there, continuing to offer the care you came to expect from Kwan-Yin Healing Arts Center.

For former patients:

Your medical records are being kept with Nourishing Life Collective, and you may request them there. For ease of access, Nourishing Life Collective is keeping the same phone and fax number: 503-701-8766 (phone) and 503-241-5484 (fax).

You may also download your medical records through the Kwan Yin Athena Portal. Simply log on, click My Health → Health Records → All Time, and select either Download or Email
